To help familiarize you with our office, we have listed some of our office policies below. If you do not find what you are looking for here, please feel free to

PATIENT REGISTRATION
We need to gather some basic information about your child when seeing them for the first time. Please complete the patient registration forms using the link below. If you need us to obtain medical records from a previous physician, please also download the medical records release form.
When coming to our office for the first time, be sure to bring the following items:
- Completed patient registration forms
- Completed medical records release form (if needed)
- Immunization history
- List of current medications including over the counter medications
- Insurance Card
- Photo ID of parent
APPOINTMENTS
We see patients by appointment. If you are unable to keep your regularly scheduled appointment, we ask that you notify us within 24 hour so that we may accommodate another child who needs to be seen.
If your child has an urgent need, we encourage you to call our office and we will do our best to accommodate you the same day. For medical “emergencies”, please go straight to the nearest emergency room or dial 911.
FINANCIAL POLICY
All co-payments, deductibles and non-covered services are due at the time of service. We accept cash, checks, debit and credit cards. We also accept HSA (health or flexible spending savings account) credit cards.
We do accept Health Choice patients who are members of United MCO, MedStar Family Choice, Amerigroup as well as Priority Partners.
Travel vaccinations must be paid for at the time of service. An itemized statement will be provided to you which should be adequate to receive reimbursement from your insurance company.
PRESCRIPTION REFILLS
When you need a refill on a medication, please contact our office at least 48 hours in advance. This gives Dr. Powers time to review your child’s chart to determine if a refill is appropriate or if your child needs to come in for a follow-up or lab work, etc.
When calling, have the following information available:
- Your pharmacy’s name and phone number
- The name of the medication(s) you need refilled
